This was another Free with MIR I found at Rite Aid the other day and forgot to post. I bought this a few months back and already received my check. The rebate ends 12/31/09 so be sure to buy this and send in the form soon.
I found this at Rite Aid, but they might still have it at Walgreen's or CVS.
